Recording only what shipped, merged, or was decided is the right filter, but it makes reverts the hard case. A PR merges Monday, the wiki calls it the current state, and it gets reverted Thursday after a bug report that lives in a ticket. Docs drift exactly like that on products I've shipped. Does the dreaming task treat a revert as a new decision that rewrites the old entry, or append a correction and leave agents to work out which one is current?

This maps to something I saw building internal tools last year: the bottleneck isn't reasoning, it's context loss at every handoff between systems. Slack-native AI works because Slack already won the 'everyone's in one place' problem, but the second you're touching a CRM, a design tool, and a codebase in the same task, you're re-explaining state to each one. Feels like the real unlock is a shared state layer underneath the apps rather than a smarter chat window on top of them, curious if that's the direction you're headed.
